Please click on the Solicitation Request Form link and type the correct information in the boxes, print out the page using your browser, and mail the printed copy to LI Bus, 700 Commercial Avenue, Garden City, NY 11530 or fax to 516-794-8670.

You MUST include both the solicitation number and description.

Suggestion: Instead of typing in the required solicitation information, you can go to Current Solicitations, and "block" the solicitation number or description and "copy" it into the appropriate box on this page.
